An exception log should help staff resolve a problem. It does not need every personal detail that appeared during the conversation. Collecting more information can make the log harder to use and broaden access unnecessarily.

Use a transaction reference, a concise description, the responsible person and the next action. Keep sensitive supporting records in the approved location and limit access according to the work involved. Distinguish the customer’s statement from the employee’s interpretation.

Review closed items and apply the business’s retention process instead of keeping every note indefinitely by habit. The operational objective is continuity and learning: what happened, how it was handled and which recurring problem deserves attention. A disciplined record helps the next employee respond accurately without turning a routine service issue into a collection of unrelated personal information.
